All Permit applicants and all Associate User applicants must submit one of the following forms:

• Radiation Worker Registration - RSD-10
• Radiation Worker Registration for ARS employees – RSD-10ARS

All Permit applicants must submit one of the following forms:

• Application to Use Radiation Producing Sources - RSD-20
• Application to Use Radiation Producing Sources for ARS employees - RSD-20ARS

LRPO applicants at ARS locations must also submit the following form:

• Application for Location Radiation Protection Officer at ARS locations - RSD-50ARS

Permit applicants must also submit the applicable forms from the list below:

• Application for Unsealed Radioactive Material Use - RSD-21
• Application to Use Nuclear Gauges - RSD-22
• Application to Use Electron Capture Detectors - RSD-23
• Application to Use Self-Shielded Irradiators - RSD-24
• Application to Use X-Ray Producing Equipment - RSD-25

Permit applicants must also submit the applicable forms from the list below, for sealed source equipment or x-ray producing equipment, that is new to the data-base:

• Sealed Source Inventory Record RSD-28
• X-Ray Producing Equipment Inventory Record - RSD-29

Dosimetry badges are required for irradiator and nuclear gauge users and may be recommended for some other applications. Area monitors are required for x-ray equipment users. To add or change dosimetry, submit the following:
